58 year old female patient with hypertrophic and hyper pigmented scar

This is a case of  58 year old female patient with Chief complaints of 


1.Hyperpigmented and hypertrophic scar on left side chest region.


HOPI: 

Patient was apparently asymptomatic 10 years ago, then developed a small pimple in the chest region and she took homeopathy treatment for 2years i.e from 2011 to 2013 
Later from 2016 it progressed further again with discharge of white sebum 
Later in 2017 they consulted the doctor as the lesion progressed in size
Treatment was given, then the lesion started decreasing in size for about 5-6 months, but later in 2018 the lesion again started to increase.
13/4/18 - drained the cyst by consultant.

After draining the scar they were asked to visit for another session, but couldn’t due to COVID
In 2022, the size of the lesion started increasing again and she developed itching on exposure to sun and also during sweating 


PAST HISTORY: 

-N/K/H/O of DM,HTN,EPILEPSY,CAD.

PERSONAL HISTORY: 
pan : 5-6 times
Diet - rice (lunch and dinner) 
non veg six times a week.

Orthopaedics:

History of fall 1 day ago leading to swelling and restriction of movement around the right ankle 








Dermatology: 








Diagnosis: 
Keloid 



Treatment: 

1. Inj. Triamcinolone 40mg / 1mg 
Intralesional steroid/stat

 A 33yr old male , who is shopkeeper by occupation  C/O •alcohol consumption since 10 years (360 ml whisky per day) • Tobacco consumption since 10 years (2 judebail per day) Patient was apparently asymptomatic 10 years back then he started consuming whisky 90 ml per day and gradually increased to 360 ml per day. Financial stressors present, eye openers present. Patient experiences sleep disturbance, tremors, fear, palpitation when not consuming alcohol. Patient is aware about harmful effects of alcohol but consuming despite of awareness. Patient started having judebail 10 years back half back per day and increase it to two packs per day.
